What is Together Connect?
Together Connect is a collaborative network of organisations and individuals, developed from our Community Champions initiative. Its purpose is to work smarter, connect services, and create better outcomes for local people.
At its heart, Together Connect aims to better understand the needs of local people and ensure they are connected to the services and support that can make a real difference in their lives. By building strong partnerships, we can reduce duplication, share resources, and support people in the places they feel safest and most comfortable.
Why It Matters
We recognise that many people and even organisations are not always aware of the support available locally, and that’s where Together Connect can make a difference. By bringing services and communities together, we can create meaningful connections that break down barriers, improve access to care, and empower people to live healthier, more independent lives.
How Together Connect Works
Together Connect brings services and communities together to create meaningful change. We aim to:
- Connect the right people to make things happen where they matter most
- Identify opportunities and share insights to ensure resources are used effectively
- Encourage early conversations to spark partnerships and reduce duplication
- Support small, impactful projects that improve health and wellbeing
Together Connect Partners
- ACT (Accessing Change Together) offers help for anyone who needs support with domestic abuse, homelessness, or substance use.
- ARCH provides confidential, free specialist sexual violence support and advocacy.
- Awayout works to prevent abuse, harm and exploitation for women, families and young people.
- Connect360 empowers legal migrants facing challenges in the Tees Valley.
- Community Pharmacy Tees Valley represents all community pharmacies to ensure patients receive maximum benefits from their medication.
- Essential Learning Curve Ltd raises awareness of trans rights, increasing diversity in community organisations, and promoting the global trans movement online.
- Everyday Language Solutions offers language support for non-English speakers 24 hours a day 365 days a year.
- Forestry England, based at Guisborough Forest, provides opportunities for people to improve their physical and mental health.
- Going For Independence CIC supports people affected by a loss of mobility or sensory impairment with advice, information and social activities.
- Here to help raise awareness of suicide for neurodivergent people and people with a learning disability.
- Home Instead Cleveland provides tailored support that helps people remain independent and safe in their own homes for as long as possible.
- Lavan Dementia Services provides specialist dementia care and support services.
- Last Hope is a community group dedicated to supporting homeless and vulnerable people in Redcar and Cleveland.
- Matrix Neurological provides practical help and support to children, young people and their families living with childhood acquired brain injury following an accident or illness.
- MIND Teesside offers a variety of support, connecting people to local organisations and or services, community groups and charities to improve their wellbeing.

- People First is an independent advocacy charity for local people, to champion fairness and equality.
- Probation Office helps people leaving prison to resettle into the community and those serving community sentences.
