Along with other Healthwatch's across the Tees Valley, we would like to know what keeps you well and how you would like to access mental health and wellbeing services in your area.

Your views will establish a baseline of local people’s knowledge and expectations of mental health and wellbeing support both now, and what they need to be in future.

Healthwatch are the independent champion for people who use health and care services. We find out what people like about services, and what could be improved, and we share these views with those with the authority to make change happen.

By sharing your views with us, we will be able to help local providers and partnerships understand what they need to do to enable you to have greater choice and control over your care, and for you to be supported to live well in your community.

We appreciate your time in completing this survey and welcome your comments and suggestions.

By participating in this survey you are allowing us to share your views with local health and social care organisations, including all local Healthwatch and Tees, Esk and Wear Valley NHS Foundation Trust.

All information will be stored anonymously in compliance with GDPR as standard.
