Why did you decide to become a Community Champion?

I work as a Care Navigator within the Community Mental Health Team in South Tees, I have forged some great links within our respective communities to enable the smooth transition between services, both in primary and secondary care and the voluntary sector for our patients. By joining the network of community champions we aim to strengthen these relationships, reaching a wider network, and to provide some insight into TEWV and community mental health care for partners new and old.

How does your role help people in South Tees?

By creating these links and nurturing those relationships we can provide better care to our patients, offering them more opportunities to better their lives, breaking down barriers to accessing services and providing them with a warm handover to other agencies that they may not access without support. We look forward to joining this network of community champions, learning together and building lasting links to benefit the people we work with and the greater community!!
